  • Color or black and white jpeg (.jpg) photos may be attached with wedding form with this form. If you have trouble attaching the photo, email your photos to news@hartselleenquirer.com.
  • Give all names in full. Avoid using nicknames and initials.
  • Please provide contact information where the applicant can be reached in case of questions.
